3.  At the nutritional consultation forms, print and complete the 3 history forms and fax to me (at least 2 hours prior to our call) at 570-421-0664. (Only complete the Confidential Case History, Coaching Agreement and 3 Day Diet Diary).

5.  (Optional) To get the most out of your time, email me a list of your questions ahead of time.

6.  I will call you and spend a full 45 minutes answering your questions.
